Summary of Incidents last week
Six incidents occurred last week.
• Hoist failure (NZ)
Action – Inspect all vacuum trucks to ensure pivot pins are secure, add spring washers and lock tight to threads. Add high tensile over tilt safety chain
• Vacuum inceptors non compliance
Action – Ensure all CR owned vacuum bins are fitted with placarding regardless as to whether they are used on the roads or not
• Parking vacuum trucks
Action – Ensure vacuum truck drivers hold valid licences. Park brakes must be applied when parking vacuum trucks. Develop parking plan for all equipment and vehicles in yards. Create toolbox talk on the subject
• Knee injury while ascending through vessel hatch
Action – Employee went to the doctor’s. Toolbox talk conducted the following day on the safe movement around vessels
• Chemical irritation to finger caused by exposure to HF acid
Action – Employee reported tingling sensation in finger to client site supervisor and CR supervisor and instructed to go to the hospital for a check up, after examination given the all clear to return to work. Toolbox talk on the subject given and recommended that as a minimum latex gloves under PVC gloves when working with alky. Signage to be placed around alky and all gloves and clothing to be cleaned prior to use
• VT031 moved from Caltex Kurnell to Caltex Lytton without notifying Asset Department. VT031 not registered by EPA QLD to operated in QLD
Action – Equipment & Logistics Planner role needs to be created for NSW
Summary of last week’s BBOs
Last week we had a total of 10 BBOs including:
2 x Hazards
1. Cardboard box on the back of a CR truck flew off the back of the truck whilst driving hitting a TPI hook truck. Toolbox talk given to remind drivers of importance of restraining loads correctly
2. AT106 considered by employees to be a hazard, due to ergonomics of getting into cab, height of vehicle and slip risk. Replacement vehicle has been requested and management to decide on future of AT106
4 x Non Conformances
1. HP unit NT036 and truck unit EDK238 checked on return from a job and found truck to be dirty and wet weather gear laying in cab, HPW equipment found laying around truck body and the equipment had not been cleaned
2. After Plant & Equipment Logbooks have been completed it takes too long for the information to be sent back to the Asset Department to get issues fixed. As a result some jobs have stopped.
3. Equipment Logbooks not being completed correctly, hours / KM incorrect, along with Asset Numbers and no name entered

Summary of Incidents last week
Two incidents occurred last week.
• Damage to cold cutting equipment
Action – Ensure all employees report equipment faults and movement forms are completed before leaving site
• Chest Pains (precautionary incident report)
Action – Employee with chest pains taken to hospital as a precautionary measure
Summary of last week’s BBOs
Last week we had a total of 9 BBOs including:
9 x Hazards
1. A nitrogen pack delivered to site was leaking from gauge
2. Ground under well sampling point blocked with large rocks and difficult to carry sampling equipment to well
3. During tank verification isolation tag found on correct valve, but during verification valve was found to be open when the tag stated the valve was closed, as a result fuel had leaked into the tank during transfer.
4. Another company setting up electric water blasting in a CR work area, where CR were conducting tube cleaning, CR operator explained to the other company representative the risks of working within a CR work area.
5. Breathing air lines were running up a ladder, but after discussion with team lines moved to reduce obstruction.
6. Catalyst and absorbent were in incorrectly labelled drum, catalyst was quarantined and reported.
7. During tank cleaning audit found that some employees were not wearing gloves whilst handling sludge.
8. Light out of test tag date, issue reported to Shell Clyde.
9. Crew preparing to clean a tank observed not wearing the correct PPE, work stopped and toolbox held to review the importance of PPE.
Please ensure when logging your BBOs that you correctly choose whether they are hazards or positive behaviour as many of the above items were logged as positive and they obviously are not. Unless you are commending people for extra ordinary behaviour please choose something other than positive behaviour when entering into the system.

Audit/Inspection Updates
During the week the following Audits /Inspections were carried out and some non conformances were found:
2 x Audits
1. Inert Entry – all areas 100% compliant.
2. Vacuum Operations – areas audited included: barricading, logbook completion, spill kits. Only non conformance was no drain cover on dock drains

Summary of Incidents last week
Two incidents occurred last week.
• Split weld on vacuum – Oxygen levels rose while vacuuming catalyst from reactor. Caused by a weld around a piece of box tubing was cracked so air was being sucked into the cyclone.
Action – Service required
• VT022 left in public area overnight – Vehicle left at base of public track / fire trail, with cabin door open.
Action – Crew were interviewed and potential hazards discussed. All vehicles must be parked in a safe and secure location at all times.
Summary of last week’s BBOs
Last week we had a total of 12 BBOs including:
2 x Hazards
1. UHP hose joiner came loose while jetting, job stopped & crew instructed on correct way to tighten joiner.
2. General public ignoring exclusion zone setup by CR. CR employee asked the public to leave the area.
7 x Positive Behavioural Observations
1. Good team work whilst using ultra high 10k pump.
2. Loading sock slipped down fitting during loading, need to find another way to attach sock.
3. Spent acid line found with no NRV or isolation valve. Line capped and operations informed of non compliance.
4. Drain grate damaged allowing gravel to wash down and block drain. Reported and replacement grate ordered.
5. Trucks can’t empty load completely due to angle of ground. Crushed rock being sourced to raise ground.
6. 3rd party requested foam fill tyres, on picking tyres up noticed this had not been done, tyres filled before leaving the supplier.
7. Hired forklift and it arrived leaking diesel from filter. Filter tightened and hire company informed.

Audit/Inspection Updates
During the week the following Audits /Inspections were carried out and some non conformances were found:
3 x Audits
1. Mobil Altona – Critical inert entry procedures audited, 100% compliant.
2. Petty Cash Reimbursement – Six areas audited, two corrective actions regarding completing travel diaries and cash advances.
3. HPW at Invercargill – Correct PPE worn throughout the project, 100% conformance.
1 x Inspection
Inspection at Caltex Kurnell – Only two extinguishers in date, eleven out of date, and four extra required to be mounted in containers or vehicles. Three extinguishers had no tags.

Summary of Incidents last week
Two incidents occurred last week.
• Canopy window breakage – Unsecured equipment moved within canopy whilst in transport damaging drivers side of vehicle.
Action - Ensure all loads are secure whilst in transport
• Ultra high hose split – Whilst running up HP022 water started to leek from the hose near the head of the pump, hose still in test date.
Action – Ensure that all hoses are inspected before use even if tagged as in date.
Summary of last week’s BBOs
Last week we had a total of 11 BBOs including:
4 x Hazard
1. Floor in grit blasting booth in Darwin is starting to breakdown and needs replacing
2. The veranda around the edge of the stores in Darwin is slippery when wet
3. Air leak in gun airline, job stopped immediately, leak fixed and recommenced job
4. Found wheel nuts loose on NV025, nuts tightened and to correct settings
4 x positive behavioural Observations
1. Good team work whilst cleaning 31F1 boiler
2. Wearing of camel packs in 43 degree heat to keep hydrated
3. Wearing of correct PPE
4. Good record keeping during the loading phase of reactors

Audit/Inspection Updates
During the week the following Audits /Inspections were carried out and some non conformances were found:
Audit of coatings procedures at Port Adelaide, resulting in a conformance score of 108 out of 125, only a few minor areas to review and improve.

Summary of Incidents last week
Two incidents occurred last week.
• Employee suffered back pain when getting out of bed while on site in remote location and,
• An employee subbed Toe while off shift on a FPSO.
The latter incident is a repeat of a more serious injury that occurred last year on an offshore platform in WA which is disappointing.
It is imperative that proper foot ware is worn at all times when on any marine installations regardless of whether on or off shift.
All employees must take the utmost of care when in remote locations and offshore as any incident or injury can cause significant logistical and operational disruption and financial loss for fellow employees and the company if a complete crew cannot be mustered for a shift.
• Last week’s BBOs
Last week we had a total of 6 BBOs including:
2 x Hazard
1. Employee noticed a chip in the windscreen when driving to site. On completion of the day’s work the chip had turned into a crack. Windscreen replaced.
2. An employee noticed that the on button had a malfunction on an interface gauge and notified the supervisor straight away. Good pickup

Audit/Inspection Updates
During the week the following Audits /Inspections were carried out and some non conformances were found:
• Inspection of job set up and crew operations on Tank 611 found to be progressing in a safe and timely manner with good communication skills and job understanding being exhibited by all team members.
• Two crew members tasked with cleaning open drain on main road of Shell Clyde refinery assessed the risk of working on an unprotected edge and used scaffold to make entry to and from drain. It was also noted that this is a highly visible job and high traffic area, in this case all the road closure permits were signed off and adequate signage and traffic controls were implemented.
• On inspection of HP019, an employee noticed that there was no test date tag on the hose from the pressure gauge to the boiler. Remove hose and test and tag.
• While conducting an LPO on a coatings job at Yarraville, the crew showed good demonstration of house- keeping also noticed by the client. The correct P.P.E was worn for the task at hand. The M.S.D.S sheets were at the job for the paint being used.

Summary of Incidents last week
Two incidents occurred last week.
•Vacuum Truck Seal Water Tank full of product
•Product left in Truck when sent for Maintenance Truck delivered to workshop for PTO mods and system check. During the system check required to open rear door. On doing this unknown contents spilled from the truck onto floor. A tub was in place in case of minor contents but could not capture volume of waste left in vessel. Toolbox to site/s importance of demob before letting equipment leave site. Discipline site management on the importance of preventative maintenance and presenting equipment clean and on time for planned maintenance tasks. Historically this has very poor.

Last week we had a total of 3 BBOs including:
1 x Hazard
1.Working on top of pontoons they have no hand rail

Audit/Inspection Updates
During the week the following Audits /Inspections were carried out and some non conformances were found:
•Lifting cradle on 5 Lance has 4 wire slings that have no test tags Have Lifting Cradle slings tested and tagged
•2mtr long delivery hose in HP040 does not have a asset number or test tag, This item was changed during the Caltex Lytton shut down Have tag and asset number sent to attach
•Audit on process to suck out redundant underground heating oil tank in Transfield workshop to decant into gully sucker.
•Skim surface of interceptor bays using vacuum hoses skimmers and vacuum truck
•EPA/DG compliance checklist on VT031. 6 non conformances detected from checklist. Confirm that waste tracking forms are available and used for truck. Broom and First aid Kit Missing from truck – Replace Level Sensor's OK but no dip-stick. Send dipstick for truck. White Equipment checklist forms not being returned to Adelaide- Ensure forms are returned regularly. Replace chain on rear valve cap. Missing CR Vacuum Warning signs - replace as required
